Corrective Actions Required: Ensure staff implement policies related to notification of injury to a child to parents. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On September 24, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: staff would review policy related to child injury notification and parent communication. Manager would review the timelines for notifications to parents. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake August 6, 2025. Through the investigation process, Licensing determined that policies outlined by the facility related to notification of injury of a child to parents was not followed.

Corrective Actions Required: Ensure that parents are notified immediately if child becomes injured. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On September 24, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: Policy related to injuries would be updated to state parents would be called without delay and staff would review the updated policy. The corrective action plan was reviewed and accepted by Licensing.

Action/s Required by Licensing: On September 24, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: a checklist would be created for reportable incidents and Manager would complete incident reports within 24 hours and submit to Licensing. The corrective action plan was reviewed and accepted by Licensing.
