Observations: Licensing toured the facility and observed that the railings to the upstairs were worn down with paint peeling.

Observations: Licensing audited the care plan's of two person in care. One person in care, who was at risk for missing/wandering, did not have a plan to locate the person in care if they went missing in the care plan.
