3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment)

Corrective Actions Required: Ensure that all outdoor play materials accessible to children are safely constructed and free from hazards, including entanglements.

Observations: Licensing observed two entanglement hazards on the top left and right sides of the blue slide in the designated play area. This represents an immediate risk to the health and safety of persons in care and must be corrected immediately. The Manager provided a verbal health and safety plan which indicated that this area of the designated play area would not be accessed by children until the entanglement hazards were repaired. The Manager submitted a work order to their maintenance department during the inspection. Licensing accepted this health and safety plan. Full compliance will not be achieved until the plan has been implemented and completed.

Observations: Licensing observed that there was hardpan (no pea gravel) at the base of the two slides (green and blue) in the designated play area. Also observed that there was ample quantities of pea gravel along the edges of the play yard available to redistribute. During the inspection, the Manager submitted a work order to the facility maintenance department to remedy this situation.
3080 - CCLR 11(4)
Description: A licensee must display in a prominent place in the community care facility the certificate of any educator or assistant employed to work in the community care facility.

Observations: Licensing reviewed 21 employee files and observed that one educator's certificate was not posted in the facility. Manager corrected this during the inspection by posting the employee's certificate on the staff bulletin board in main foyer.
