Corrective Actions Required: Ensure that the community care facility is operated in a manner that promotes the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 26, 2025, Licensee submitted a corrective action plan to Licensing. This corrective action plan included: all care plans would be reviewed and updated with each review date as needed and if changes occurred and reviewed with staff, and oral care plans would reflect specific and accurate oral care needs and goals .The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake October 20, 2025. Through the investigation process, Licensing determined that a person in care's oral health care plan did not include specific direction on how staff provided oral health care or that the person in care's family at times provided some of the care.
