Observations: Licensing observed that four fire extinguishers has last been serviced since February 2023 and had a service date of February 21, 2024.
3000 - SPS DOLSOP (Depth)
Description: Depth guidelines for loose fill material (if protective surfacing used) :
minimum 15 cm (6”) for 1.5 metre (5’) equipment height
minimum 30 cm (12”) for 2.3 metre (7’) equipment height
(Consider surfacing table, Triax, etc.)
Corrective Actions Required: Ensure there is at least 6" of protective surfacing for equipment that's is 5 ' in height.

Observations: Licensing observed that surfacing under equipment measuring 5' in height was 1" under the slide section of the equipment.
3010 - CCLR 19(1)(c)
Description: A licensee must not employ a person in a community care facility unless the licensee or, in the case of a person who is not the manager, the manager has first met with the person and obtained all of the following: a record of the person’s work history
Corrective Actions Required: Ensure a record of a persons work history is obatined for a person employed at a community care facility.

Observations: Licensing audited four staff files, one file was missing work history.
3080 - CCLR 11(1)
Description: A licensee must display in a prominent place in the community care facility both the licence, including any terms or conditions of the licence, and the name of the manager.
Corrective Actions Required: Ensure a certificate for an educator or assistant is posted.
