Observations: Licensing reviewed all nine persons in care's nutrition care plans. Staff stated one person in care had received full dentures in 2024 and the care plan stated the person in care did not have many teeth, could not chew their food and required minced/soft/pureed cut up food. Staff stated the person in care was on a regular texture diet since February 2024. The second nutrition care plan stated a person in care required a soft minced diet to be cut in small pieces. Staff stated that the person in care was on a regular diet with small cut pieces of food since 2021.

Observations: Licensing observed the menu substitution list for 2024. Staff stated there was no menu substitution list for 2025 and that staff had to make one food substitution on January 1, 2025 for the dinner meal.
