Description: A licensee must operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Corrective Actions Required: Ensure that the facility is operated in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 13, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the Dietician would review all diet orders to ensure no mixed texture diet orders, new diet books were created for each servery which included one person in care per sheet and showed each meal separately, white boards were mounted to each servery to indicate any diet changes made by the Dietician for a three week period, and all person in care's diet orders will be reviewed by June 17, 2025, food service workers and care staff were to be re-educated on diet textures and dining service best practices by June 24, 2025, and dining room audits will be conducted monthly ongoing. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake dated May 26, 2025. Through the investigation process, Licensing determined that staff had not promoted the health, safety and dignity of a person in care. Specifically, staff having the skills and abilities necessary to carry out the duties assigned.

Description: A licensee must ensure that the care and supervision of a person in care is consistent with the terms and conditions of the person in care’s care plan.

Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention.
