Observations: Entanglement test device detected an entanglement on the left and right side of the slide. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. Licensee Contact stated they would block access to the slide until the entanglement is fixed. Full compliance will not be achieved until the plan has been implemented and completed.
