Observations: Licensing observed that the fire extinguisher was last serviced on October 2022.

Observations: Licensing observed the slide that measured two feet in height was missing a bolt that secured the slide to the base. The slide had been temporarily taped with duct tape to secure it in place. Staff stated children would be supervised when the slide is in use and there was a plan for it to be repaired.
