Observations: Licensing toured the facility and observed that in both bathrooms and the laundry area, there was missing drywall with exposed pipes and wood, with insulation exposed.

Observations: Licensing audited three staff files and observed that all three staff did not have performance reviews completed in the past year, and kept on file, as per policy.
