Observations: Licensing reviewed past attendance records and observed that on four occasions two children were missing departure times on Sept. 8, 2022. And on September 9, 2022 one child was missing arrival time and a different child was missing departure time. On Sept. 12, 2022 two children were missing departure times. On September 13, 2022 one child was missing departure time.

Description: A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact
