Observations: Licensing toured the facility and noted scrapes and gouges out of the wall in a common area on the seventh floor, and marks on the floors and scrapes on the wall in rooms of persons in care on the seventh and fifth floors.

Observations: Licensing audited 26 staff files. The Director of Care stated the employees were recent hires and should have had a performance review done within a probationary year. Five of the 26 staff audited did not have a performance review.

Observations: Licensing audited the Narcotic record count binder on three floors. Multiple dates in March 2023 did not have Narcotic counts completed and signed by staff.
