Observations: In the outdoor play area Licensing observed that a teeter totter was missing a safety rubber cover exposing a screw and creating a pinch hazards that extended and contracted with the movement of the equipment.
3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment)
Use Entanglement test device
Corrective Actions Required: Ensure that equipment is free of entanglement hazards.

Observations: Licensing used entanglement tool and detected an entanglement hazard on a the left hand side of a slide measuring five feet in height. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. Manager stated the slide would not be in use until entanglement hazards is mitigated.
3040 - CCLR 53(1)(a)
Description: If a licensee has agreed with a parent to give a child any medication prescribed by a medical practitioner or provided by the parent, the licensee and his or her employees must ensure that the medication is (a) administered to the child in the amount and at the times specified by the child’s parent or in the child’s record or care plan

Observations: Licenisng observed that a medication form indicating that medication had been administered to a child in the month of July 2023, did not contain a record specifying the amount or time the medication should be administered as prescribed by a parent or medical practitioner.
