3070 - CCLR 57(1)(a)
Description: A licensee must keep current records for each child showing (a) the information set out in subsection (2)
Corrective Actions Required: Ensure all children under the age of 12 years and under the supervision of the Licensee have a full registration package on file, if the child is present during the operating hours on the Licence.

Observations: Licensing observed a ten year old child was present as a playdate with Licensee's own child. This child did not have any registration information on file.
3070 - CCLR 57(2)(a)
Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.
Corrective Actions Required: Ensure all children have their sex recorded on file.

Observations: Licensing reviewed five children's files. Three children did not have child's sex on file. Licensee has created her own child's registration form which does not have a space for parents to record child's sex.
3070 - CCLR 57(3)(a)
Description: A licensee must have in writing from a parent, and maintain at the community care facility, consent (a) to call a medical practitioner or ambulance in case of accident or illness if the parent cannot immediately be reached,
Corrective Actions Required: Ensure all children's registration records show the parent/s have given permission to call an ambulance in case of accident/illness if the parent cannot immediately be reached.

Observations: Licensing reviewed five children's files. Five children's registration forms did not have parent's consent to call an ambulance if needed.
