Corrective Actions Required: Ensure that supply equipment, furniture and supplies are not located so as to block or hamper an exit in the case of fire or other emergency. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 9, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: egress was cleared on December 17, 2025 and would be kept clear with daily safety sweeps and access would be monitored by leadership. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake dated December 16, 2025. Through the investigation process, Licensing determined that an emergency exit had been blocked with construction equipment and supplies.

Corrective Actions Required: Ensure that children do not have access to any objects or substances that may be hazardous to their health and safety. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 9, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: on December 17, 2025, temporary physical barriers and protective covers had been installed to exposed hardware and construction materials. Permanent installation, re-adhesion and fixation of panels, baseboards and sink would be completed by February 5, 2026. The corrective action plan was reviewed and accepted by Licensing.
