Description: A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Corrective Actions Required: Ensure a reportable incident described in Schedule H of Child Care Licensing Regulation, which a child is involved in, or may have been involved in, must notify Licensing within 24 hours. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 1 to June 2, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: An incident would be notified to the Licensee Contact by the manager within one hour of becoming aware of, and to be notified Licensing within 24 hours as described in Schedule H of Child Care Licensing Regulation. An incident tacking log would be recorded by the Licensee Contact, and the manager's training regarding reportable incidents would be provided to Licensing on June 26, 2026. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake dated May 13, 2026. Through the investigation process, Licensing determined the manager was aware of an incident on April 21, 2026, in which a staff had a physical response toward a child, and conducted an internal investigation; but did not notify Licensing within 24 hours after a child is involved in, or may have been involved in a reportable incident described in Schedule H.
