Description: A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are safely constructed, free from hazards and in good repair.
Corrective Actions Required: Ensure all outdoor play materials/equipment accessible to children are safely constructed/free from hazards/in good repair. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 7, November 10, and December 4, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: The gaps in between the rubber tiles would be filled by fine gravel/sand mixture by November 30, 2025. The damaged rubber tile had been replaced on October 20, 2025. The corrective action plan was reviewed and accepted by Licensing. On January 6, 2026, Licensing observed the damaged rubber tile had been replaced, and the gaps in between the rubber tiles had been filled by gravel; however, the gaps were not filled to eliminate the potential hazard.

Observations: Regarding intake October 17, 2025. Through the investigation process, Licensing determined that the outdoor ground surface accessible to children was not in good repair that gaps in between the rubber tiles with approximately 1.5 to 2 inches at few areas, and a hole in the rubber tile was measured approximately 3 inches in depth and 5 inches in width.

Corrective Actions Required: Ensure to operate the community care facility in a manner that will promote the health, safety, & dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 7, November 10, and December 4, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: All Staff would be reminded to conduct a daily inspection of the playground, to record if any damage on the playground, and report to the manager. The manager will be monitoring if any rubble tile gaps created. The corrective action plan was reviewed and accepted by Licensing.
Corrective Actions Taken By Licensee: On January 6, 2026, Licensing observed an email sent to staff on October 27, 2025 to conduct daily inspections of the playground and to complete a log of the playground inspections to be kept on file.
Actions Required By Licensing: No further action required; this contravention has been addressed.
