Corrective Actions Required: Ensure to operate the community care facility in a manner that will promote the health, safety and dignity of persons in care, specifically, staffing skills. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 13, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: On November 28, 2025, management would reaffirm expectations regarding staff conduct, supervision and professional communication through a focused Professional Development, 8 hours session. Managers would conduct bi-weekly classroom check ins to ensure practices align with their policies until February 2026. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake October 7, 2025. Through the investigation process, Licensing determined that staff did not demonstrate skills to promote health, safety and dignity of children in care when they did not respond promptly to children's severe illness symptoms, follow and implemented procedures and policies, notify parents and Licensing of reportable incidents.

Corrective Actions Required: Ensure that policies and procedures are implemented by employees. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 13, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: On November 28, 2025, they would conduct a comprehensive policy refresher to review all the procedures related supervision, communication and reporting. Staff would sign off acknowledging their understanding and agreement to follow policies as written. Manager would conduct monthly compliance reviews of incident documentation and communication logs until May 2026. The corrective action plan was reviewed and accepted by Licensing.

Corrective Actions Required: Ensure that parent/emergency is notified immediately if child becomes ill or is injured. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 13, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: staff would be retrained on existing Illness and Injury Notification Policy to ensure immediate parent contact occurs both verbally and in writing on November 28, 2025. Supervisors would review each incident report before the end of the day to confirm that parent notification was completed. Management would conduct weekly audits weekly for three months until February 2026. The corrective action plan was reviewed and accepted by Licensing.

Corrective Actions Required: Ensure the Medical Health Officer is notified within 24 hours if a child has been involved in/ may have been involved in a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 13, 2025, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: Staff and management would be retrained on Schedule H reportable incidents and the 24-hour notification requirement on November 28, 2025. A Reportable Incident Log would be maintained and checked daily by the manager to verify timeliness of reporting. The Regional Manager would review all reports weekly to confirm compliance. The corrective action plan was reviewed and accepted by Licensing.
