3000 - SPS DOLSOP (Entanglement)
Description: Equipment is free of entanglement hazards in areas of uncontrolled motion. (Something around the child’s neck becomes caught on equipment) Use Entanglement test device

Observations: Licensing used the entanglement device and detected entanglements on both sides of the blue slide in the toddler play area. This represents an immediate risk to the health and safety of children in care and must be corrected immediately. Staff stated that children will not use the slide until the entanglement can be corrected and tested to be free of entanglement.
