Observations: Licensing observed the elevated equipment in the designated outdoor play area. This equipment appeared to need cleaning and maintenance. Licensee stated she does not use her designated outdoor play area between approximately October and April due to the space being too muddy.

Description: A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.

Corrective Actions Required: Ensure all children in care are actively supervised by a qualified responsible adult who has all the required employee paperwork in place.

Observations: On arrival Licensing observed two seven year old children outside in the front driveway, one on a bike the other on a scooter. Licensing did not observe anyone actively supervising the children. During the inspection the two seven year old children went upstairs for approximately 30 minutes to watch TV with Licensee's husband. One of these children was the Licensee's grandchild, the other was a child in care.

Description: A licensee must keep, for each child, a record showing the following information: (a)name, sex, date of birth, medical insurance plan number (MSP number does not apply to REC only) and immunization status.

Corrective Actions Required: Ensure all children enrolled have a Medical Health Number on file.
