Corrective Actions Required: Ensure that a person in care is not removed from the community care facility by any person except the person in care's parent or representative, or a person authorized in writing by that person. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 27, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a private visitor was banned from the facility and staff would be reminded to ensure only designated persons identified may remove persons in care from the facility via memo by December 7, 2025. The corrective action plan was reviewed and accepted by Licensing.
