3030 - CCLR 52(1)(b)
Description: A licensee must ensure that a child, while under the care or supervision of the licensee, is not subjected to confinement or physical restraint by an employee, except as authorized in a child care plan if the plan includes instructions respecting behavioural guidance.
Corrective Actions Required: Ensure a child is not subjected to confinement or physical restraint by an employee except when authorized in a child's care plan. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 14, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan indicated that restraints will not be in use unless a care plan was in place an staff had appropriate training. A training was provided to all employees on March 14, 2025, to review this as well as de-escalating techniques and alternative strategies of guidance. This will also be added as part of orientation training with all new employees.
The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake February 13, 2025. Through the investigation process, Licensing determined that a staff member had picked up children and moved them without the children's consent, and both children had resisted this restraint.
3070 - CCLR 55(2)(a)
Description: A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Corrective Actions Required: Ensure notification occurs within 24 hours when a child is involved in a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 14, 2025, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: If an emergency restraint was necessary the facility would notify Licensing within 24 hours and an incident report would be completed. This information had been added t o new employee orientation training and was to be reviewed with all employees on March 14, 2025. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake February 13, 2025. Through the investigation process, Licensing determined that a staff member had used an emergency restraint to stop the child from running into traffic, this emergency restraint was not reported to Licensing.
