Observations: Licensing observed a child was involved in an incident in February 2026 that met the definitions of Schedule H, that was not reported to the Medical Health Officer.

Description: Applicants for licences and licensees must notify a medical health officer immediately of any change in the information provided under section 9
Corrective Actions Required: Ensure Licensing is notified of any changes of information provided under section 9.

Observations: Licensing observed the Manager on the licence was no longer employed at the facility. A new Manager was in place and it had been more than 30 days since the Manager changed. Licensing was not notified of the change.
