Description: A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Corrective Actions Required: Ensure that staff maintain continuous supervision for persons in care at all times. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 16, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: all staff received an updated supervision policy that all staff signed that they had read and understood the policy, staff would continually move around the outdoor play space, maintain clear sightlines to respond quickly. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake May 19, 2026. Through the investigation process, Licensing determined that supervision by staff was not maintained at all times, specifically on the outdoor play space.

Description: A licensee must ensure that a child, while under the care or supervision of the licensee, is not subjected to shoving, hitting or shaking by an employee or another child, or confinement or physical restraint by another child
Corrective Actions Required: Ensure that children are not subjected to physical restraint by another child while supervised and in care at the facility. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 16, 2026, Licensee Contact submitted a corrective action plan to Licensing. The corrective action plan included: staff actively scanning the entire play space, positioning themselves strategically, and anticipating potential risk before they escalate and continually move around the outdoor play space to maintain clear sightlines and respond quickly if something happens. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake May 19, 2026. Through the investigation process, Licensing determined that a child had been physically restrained by another child when a child put another child in a choke hold while under the care and supervision of the Licensee.

Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 16, 2026, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: incidents would be reported to Licensing within 24 hours of a reportable incident. The corrective action plan was reviewed and accepted by Licensing.
