Observations: Licensing toured facility and observed in living area a recliner that was in poor condition, stuffing coming out, ripped cover.

Corrective Actions Required: Ensure laundry area has a slip resistant flooring.

Observations: Licensing toured the facility and observed that the laundry area did not have a slip resistant flooring. Persons in care do assist with laundry.

Corrective Actions Taken By Licensee: Licensing Officer reviewed care plan documents that were emailed to Licensing and they included height and weight of persons in care.
