Corrective Actions Required: Ensure to notify the medical health officer within 24 hours after a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 6, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: all staff and management will receive training and ongoing support on incident identification, documentation, notification requirements, and reporting timelines, with clear reference materials available, immediate notification and parent communication procedures in place, and all reportable incidents reported to Licensing within 24 hours. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake May 29, 2026. Through the investigation process, Licensing determined that the facility records identified multiple reportable incidents involving aggressive or unusual behaviour that were not reported to Licensing within 24 hours.

Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On July 6, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: care plans will be developed promptly in consultation with families when a child demonstrates unusual or aggressive behaviour, reviewed regularly to ensure they reflect current needs, and updated as required. The corrective action plan was reviewed and accepted by Licensing.
