Action/s Required by Licensing: On July 6, 2026, Manager submitted a corrective action plan to Licensing. The corrective action plan included: all staff and management will receive training and ongoing support on incident identification, documentation, notification requirements, and reporting timelines, with clear reference materials available, immediate notification and parent communication procedures in place, and all reportable incidents reported to Licensing within 24 hours. The corrective action plan was reviewed and accepted by Licensing.

Observations: Regarding intake May 29, 2026. Through the investigation process, Licensing determined that the facility records identified multiple reportable incidents involving aggressive or unusual behaviour that were not reported to Licensing within 24 hours.
