Description: A licensee must ensure that all indoor and outdoor play materials and equipment accessible to children are safely constructed, free from hazards and in good repair.

Observations: Licesning observed the following three items requiring repair on the elevated composite play structure: (1) On the climbing ramp, the two lowest blue footholds are loose (2) On the long green handrails, left and right, of the climbing ladder are loose (3) On the protective barrier with the green steering wheel, the top rail is warping and becoming loose from the vertical slats in multiple locations.
3000 - SPS DOLSOP (Depth)
Description: Depth guidelines for loose fill material (if protective surfacing used) :
minimum 15 cm (6”) for 1.5 metre (5’) equipment height
minimum 30 cm (12”) for 2.3 metre (7’) equipment height
(Consider surfacing table, Triax, etc.)
Corrective Actions Required: Ensure depth guidelines for loose fill material is minimum six inches for equipment up to five feet in height.

Observations: Licensing tested protective surfacing depth (pea gravel) in multiple locations in the fall zone of the composite elevated play structure measuring fifty five (55) inches in fall height. Licensing observed there was zero to two inches of depth between the slide and the climbing ramp, with hard pan visible in the fall zone.
3070 - CCLR 56(1)(f)
Description: A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Corrective Actions Required: Ensure a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents as described in Schedule H is kept current.
