Observations: Licensing audited five children's files and observed five did not have a record of any person not permitted access.

Corrective Actions Taken By Licensee: Licensing audited ten children's files and observed ten files had persons not permitted access added to the files.
Actions Required By Licensing: No further action required; this contravention has been addressed.
