A licensee must keep, for each child, a record showing the following information: (d) name and telephone number of a parent, medical practitioner (doctor's name and number does not apply to REC only) and emergency contact
Observation: Through the investigation process, Licensing determined that the Licensee had not kept a record showing the name and telephone number of a medical practitioner for each child.
Corrective action: Ensure a record was kept for each child showing the name and telephone number of a medical practitioner. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 28, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included delegating a staff to be responsible for the collection of children's information and ensuring this information was added to the children's files. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. Follow Up Inspection required. Follow Up Inspection on July 13, 2023 identified that children's files were complete.

Observation: Through the investigation process, Licensing determined that the Licensee had not kept a record of immunization status for each child.
Corrective action: Ensure a record was kept for each child showing immunization status. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 28, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included delegating a staff to be responsible for the collection of children's information and ensuring this information was added to the children's files. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. Follow Up Inspection required. Follow Up Inspection on July 13, 2023 identified that children's files were complete.

Observation: Through the investigation process, Licensing determined that the Licensee had not kept a record of any person who is not permitted access to the child.
Corrective action: Ensure a record is kept showing any person who is not permitted access to the child. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 28, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included delegating a staff to be responsible for the collection of children's information and ensuring this information was added to the children's files. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. Follow Up Inspection required. Follow Up Inspection on July 13, 2023 identified that children's files were complete.
Without limiting subsection (1), a licensee must ensure that the ratio of employees to children attending a community care facility is no less than that permitted in Schedule E.
Observation: Through the investigation process, Licensing determined that the Licensee had not ensured that the ratios of employees to children in care were maintained.
Corrective action: Ensure that the ratio of employees to children attending a community care facility was no less than that permitted in Schedule E. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 28, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included hiring qualified staff and reducing hours in the Group Child Care (Under 36 Months) program if necessary to ensure staff to child ratio was met. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. Follow Up Inspection required. Follow Up Inspection on July 13, 2023, identified that staff to child ratio complied with legislation.
