3070 - CCLR 55(2)(a) 
 

A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

Observation: Through the investigation process, Licensing determined that a reportable incident was not reported to licensing.

Corrective action: Ensure the medical health officer is notified of any reportable incident within 24 hours. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 27, 2022 and July 12, 2022, Licensee submitted a corrective action plan to Licensing. The corrective action plan included reviewing the CCALA, CCLR and Schedule H of the CCLR; review of Policies and Procedures and confirmation of reading these documents. New procedures would be in place when interpersonal conflicts were identified and documented. There would be consequences in place for staff not completing the mandatory training. The corrective action plan was reviewed and accepted by Licensing. On September 13, 2022, Licensing received confirmation that the corrective action plan was completed. Licensing to review and monitor this corrective action plan.

3010 - CCALA 7(1)(b)(i) 
 

A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

Observation: Through the investigation process, Licensing determined that staff did not demonstrate the necessary skills to carry out their assigned duties.

Corrective action: Ensure the facility is operated that will meet the health and safety of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 27, 2022 and July 12, 2022, Licensee submitted a corrective action plan to Licensing. The corrective action plan included reviewing the CCALA, CCLR and Schedule H of the CCLR; review of Policies and Procedures and confirmation of reading these documents. Observation: Through the investigation process, Licensing determined that children in care were subjected to emotional abuse.

Corrective action: Ensure children in care are not subjected to emotional abuse. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: Between June 27, 2022 and July 12, 2022, Licensee submitted a corrective action plan to Licensing. The corrective action plan included reviewing the CCALA, CCLR and Schedule H of the CCLR; review of Policies and Procedures and confirmation of reading these documents.
