A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.

Observation: Through the investigation process, Licensing determined that a minor incident between children had not been recorded.

Corrective action: Ensure a record is kept of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On May 20, 2022 Licensee submitted a corrective action plan to Licensing. This corrective action plan included re-education of recording in the minor incident log would happen with staff, Licensee would follow up weekly to ensure that staff were using the log book, staff would record minor incidents and once a parent had been notified of the incident, the log book would be signed by the staff and Licensee. This corrective action plan was reviewed and accepted by Licensing. On September 23, 2022 Licensing received a document which indicated all staff had signed off that they had been educated and aware of the minor incident log book.
