A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Observation: Through the investigation process, Licensing determined that staff did not demonstrate the skills and abilities to carry out the duties as assigned. Specifically, staff left a child at the school and did not confirm if a child had previously been picked up by a family member.
Corrective action: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care, specifically regarding suitability of staff members. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On January 23, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: head counts would be completed when all children were in care before and after any transition, staff were not to leave the premises at the schools until all children on the pick up list joined the group or confirmation was received by the family that children had already been picked up and staff were responsible for checking with the school Manager regarding communication from parents about attendance. The corrective action plan was reviewed and accepted by Licensing.

A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation: Through the investigation process, Licensing determined that a reportable incident occurred on December 5, 2022 and Licensing was not notified of the incident until December 8, 2022.
Corrective action: Ensure that the medical health officer is notified within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 23, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included a staff meeting to be held on February 24, 2023 to discuss Licensing requirements around incident reporting, and Licensing received a form which indicated 10 staff had signed and understood the discussion. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. On July 27, 2023, Licensing received the staff meeting agenda.
A licensee must keep, for each child, a record showing the following information: (c) daily attendance record, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure
Observation: Through the investigation process, Licensing determined that children were not signed into care on the attendance records.
Corrective action: Ensure that a daily attendance record is kept, indicating for each day whether the child is absent or, if the child is present, the time of arrival and departure. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 23, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included the pick up list was to remain with staff and children were signed in by staff once they arrived to care. The corrective action plan was reviewed and accepted by Licensing.
