A care plan must include all of the following: in the case of a person in care who receives a type of care described as Long Term Care or who may be prone to falling, a fall prevention plan, which must address a plan for following up on any falls suffered by a person in care
Observation: Through the investigation process, Licensing determined a person in care did not have a plan in place for following up on any falls suffered.
Corrective action: Ensure a care plan includes, in the case of a person who is prone to falling, a fall prevention plan, which must address an assessment of the nature of the risk of falling presented by the person in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On January 13, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included the facilities care planning process was updated to address fall prevention, all persons in care had a completed fall assessment and their care plans reflect the change, and Licensing received an updated care plan for a person in care which addressed fall assessment, prevention and follow up. The corrective action plan was reviewed and accepted by Licensing.
