Observation: Through the investigation process, Licensing determined licensee did not operate the community care facility in a manner that will promote the health, safety and dignity of persons in care. Specifically, related to documentation for former person in care's medication and personal care.
Corrective action: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan specific to the contravention. Action/s Required by Licensing: On November 13, 2024, Licensee submitted a corrective action plan to Licensing. This corrective action plan included: On November 28, 2024, Licensing received documentation regarding education being provided to the care team to review the process and accountabilities for documentation of personal care. Education was also provided to the nursing team to review the process for documenting physician orders for prescription creams, as well as for documentation and ongoing assessment for topical treatments. This corrective action plan was reviewed and accepted by Licensing.

Observation: Through the investigation process, Licensing determined that a person in care's medication and personal care documentation was not consistently completed by staff.
Corrective action: Ensure that the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On November 13, 2024, Licensee submitted a corrective action plan to Licensing. This corrective action plan included that by November 30, 2024, education was to be provided to the care team to review the process and accountabilities for documentation of personal care and education was also provided to the nursing team to review the process for documenting physician orders for prescription creams, as well as for documentation and ongoing assessment for topical treatments. This corrective action plan was reviewed and accepted by Licensing. On November 28, 2024, Licensing received documentation which included a list of care staff signatures from those who attended the education provided.
