Observation: Through the investigation process, Licensing determined that staff did not demonstrate the skills and abilities to carry out the duties as assigned. Specifically, ensuring the safety of all children in care.
Corrective action: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care, specifically regarding suitability of staff members. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On May 4, 17 and 23, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: all staff would follow children's care plans, children would be kept under close supervision at all times and staff would document concerning behaviours, children would be allowed in the bathroom with a small group of four children or less and wait on the chairs in the hallway of the bathroom when waiting for other children to finish in the bathroom. The corrective action plan was reviewed and accepted by Licensing. On June 2, 2023, Licensing conducted a follow up inspection and observed the corrective action plan to be followed.
A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation: Through the investigation process, Licensing determined between January 16, 2023 and March 30, 2023, there were four minor incidents of aggressive/unusual behaviour that were not reported to Licensing.
Corrective action: Ensure that the medical health officer is notified within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On May 4, 17 and 23, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included all staff would be trained and reminded on what incidents were required to be reported to Licensing within 24 hours at a staff meeting on May 4, 2023, staff would immediately notify the Manager and fill out an Incident report. The corrective action plan was reviewed and accepted by Licensing. On June 2, 2023, Licensing conducted a follow up inspection and reviewed the minor incident log. It did not appear any new incidents of aggressive/unusual behaviour had occurred.
