Observation: Through the investigation process Licensing determined Licensing was not notified about a reportable incident.
Corrective action: Ensure to notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan specific to this contravention. Action/s Required by Licensing: This investigation was identified in the Final Investigation Report which identified contraventions to the Community Care and Assisted Living Act and Child Care Licensing Regulation. Conditions were imposed on the facility licence on April 26, 2023.
Without limiting subsection (1) a licensee must ensure that children attending a community care facility are divided into groups, with each group having no more children than that permitted in Schedule E, (except FCC/IHMACC/REC; child qualified if in age range or within one month (except GCC School Age)).
Observation: Through the investigation processing Licensing determined that staff to child ratios were not maintained on October 6, 2022.

A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation: Through the investigation process Licensing determined children were left unsupervised on the school property after school dismissal.
