A licensee must keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H.
Observation: Through the investigation process, Licensing determined staff had not been documenting minor incidents.
Corrective action: Ensure to keep current records of each of the following: (f) a log of minor accidents, illnesses and unexpected events involving children, that did not require medical attention and were not reportable incidents described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On June 7, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included: BGCCVI would create a new Documentation and Notification Policy and Parent Sign Off of Incident Form by July 4, 2023 and all staff would review and sign off on the new documents at the next staff meeting. The corrective action plan was reviewed and accepted by Licensing. On July 13, 2023 Licensing received evidence showing the development of the policies and Incident Form, as well as meeting minute from the June 29, 2023 staff meeting showing Incident Reporting protocols were reviewed and discussed.
A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child becomes ill or is injured,
Observation: Through the investigation process, Licensing determined that an incident involving a child hitting their head, was not reported to the parent.
