Observation: Through the investigation process, Licensing determined that a healthy and safe environment was not provided at all times while children were under the supervision of employees. Specifically, there was no thorough cleaning with the mice infestation.
Corrective action: Ensure that a healthy and safe environment is provided at all times while children are under the supervision of employees specifically ensuring a thorough cleaning is conducted for the mice infestation. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On April 30, 2024 Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the creation of a monthly cleaning calendar that included daily, weekly, and monthly cleaning tasks; daily disinfecting of high touch services(cubby area, bookshelf, play furniture, toys etc), and equipment ( tables and chairs and sink) and floors would be swept; staff were reminded of their daily cleaning tasks at the beginning of their shifts; the team lead completed daily walk throughs to the various spaces to ensure that cleaning was being done and; communication with district custodial staff are notified when any mice feces are found and it is been cleaned promptly. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan. On August 2, 2024 Licensing observed the corrective action plan implemented in the licensed programs. Licensee Contact to submit the forms being used. Manager stated: the leaders were checking and completing the weekly, daily and monthly tasks to be completed in their respective rooms; daily disinfecting was being completed of high touch surfaces by staff or School Custodians. Specified staff had completed a daily walk through of the licenced spaces to ensure the cleaning was being completed; there was ongoing communication with the School District custodians regarding any cleaning that was required and they had limited sightings of the mice. Licensing to review and monitor this corrective action plan.
