Subject to subsection (3), if a person in care is involved in a reportable incident, the licensee must immediately notify a medical health officer, in the form and in the manner required by the medical health officer
Observation: Through the investigation process, Licensing determined that the licensee did not notify a medical health officer, in the form and in the manner required by the medical health officer, when a person in care was involved in a reportable incident.
Corrective action: Ensure to notify a medical health officer, in the form and in the manner required by the medical health officer, if a person in care is involved in a reportable incident. Action/s Required by Licensee: Licensee to submit a corrective action plan specific to the contravention. Action/s Required by Licensing: On March 31, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included a memo posted in all nursing stations regarding staff direction for reporting incidents and the Director of Care reviewed all incident reports between April 1 and 13, 2023 to ensure all representatives were notified of the incidents. The corrective action plan was reviewed and accepted by Licensing. On April 14, 2023, Licensing received confirmation that the incident report audit was completed.

Observation: Through the investigation process, Licensing determined that the licensee did not operate the community care facility in a manner that will promote the health, safety, and dignity of persons in care.
Corrective action: Ensure to operate the community care facility in a manner that promotes the health, safety, and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 31, 2023, Jillian Thorne, former Director of Care (DOC), submitted a corrective action plan to Licensing. The corrective action plan included that a staff member was no longer employed at the facility; a poster was posted on the main floor and each care floor regarding Resident Bill of Rights and offered staff an online course in Memory Care approaches; a memo was posted regarding staff direction related to RCR 76(1) and RCR 77(2)(c); the DOC audited reportable incidents for April 1-13, 2023 to ensure reps were called; baths continue to be offered as per care plan. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.

If a person in care becomes ill or is injured while under the care or supervision of a licensee, the licensee must immediately notify the parent or representative, or contact person, of the person in care
Observation: Through the investigation process, Licensing determined that the Licensee did not immediately notify the parent or representative, or contact person, of the person in care, that they became ill or injured.
Corrective action: Ensure that if a person becomes ill or injured while under the care or supervision of a licensee, that the licensee immediately notifies the parent or representative, or contact person, of the person in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 31, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included a memo posted in all nursing stations regarding staff direction for reporting and the Director of Care reviewed all incident reports from April 1 and 13, 2023 to ensure representatives were informed. The corrective action plan was reviewed and accepted by Licensing. On April 14, 2023, Licensing received confirmation that the incident reporting audit was completed.

Action/s Required by Licensing: On March 31, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included that a staff member was no longer employed at the facility as of February 28, 2023, a poster was posted on the main floor and each care floor regarding the Resident Bill of Rights and staff were offered an online course in Memory Care approaches. The corrective action plan was reviewed and accepted by Licensing. On April 14, 2023, Licensing received a copy of the Resident Bill of Rights poster that had been posted at the facility.
