3030 - CCLR 51(2)(a) 
 

If the child has a care plan that includes instructions respecting behavioural guidance, the licensee must ensure that any behavioural guidance given to the child is consistent with those instructions

Observation: Through the investigation process, Licensing determined behavioural guidance strategies used with a child in care were not consistent with the care plan in place for the child.

Corrective action: Ensure that if the child has a care plan that includes instructions respecting behavioural guidance, the licensee must ensure that any behavioural guidance given to the child was consistent with those instructions. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On October 20, November 3, November 16 and November 23, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: a Children Requiring Extra Supports Policy would be developed, which would include care plans were in place for each child, who had additional needs. Additionally, management would provide staff with four modules related to working with children with challenging behaviours, which would be presented to staff by April 30, 2024. The corrective action plan was reviewed and accepted by Licensing. On November 20, 2023, Licensing received a copy of the completed policy. On February 14, 2024, Licensing received a request to extend the deadline for the four modules, due to high staff turn over, until June 30, 2024. The revised corrective action plan was reviewed and accepted by Licensing. Licensing to monitor this corrective action plan. June 28, 2024 Licensing received evidence of the four modules that were used for staff training, as well as meeting minutes showing the dates of completion.

3070 - CCLR 55(2)(a) 
 

A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

Observation: Through the investigation process, Licensing determined there were multiple incidents involving a child in care that had not been reported to Licensing.

Corrective action: Ensure to notify the medical health officer within 24 hours after (a) a child was involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On October 20, November 3, November 16 and November 23, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: A staff meeting was held on September 18, 2023, where Schedule H and Incident Reporting was discussed in detail with all staff. Additionally, the Manager would have a checklist/sign off sheet showing that they had reviewed any and all incidents daily. As part of the Site managers duties they would be required to review all incident reports by the next business day and notify Licensing within 24 hours if any were reportable. The corrective action plan was reviewed and accepted by Licensing. On November 22, 2023, Licensing received meeting minutes from the September 18, 2023, Staff Meeting showing incident reporting was discussed in detail. On November 28, 2023, Licensing conducted a Routine Follow Up Inspection and observed the Incident Reporting Checklist had been completed daily.

3080 - CCALA 7(1)(b)(i) 
 

A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

Observation: Through the investigation process, Licensing determined the Licensee did not ensure supports were in place for a child in care who required additional support.

Corrective action: Ensure to operate the community care facility in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On October 20, November 3, November 16 and November 23, 2023 the Licensee submitted a corrective action plan to Licensing.
