A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation: Through the investigation process, Licensing determined that the medical health officer was not notified within 24 hours after a child was involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee.
Corrective action: Ensure the medical health officer was notified within 24 hours after a child was involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a correction action plan to Licensing specific to this contravention. Action/s Required by Licensing: On March 28, 2022, Licensee submitted a corrective action plan to Licensing. This corrective action plan included: Staff were responsible in ensuring all incidents were reported during or after their shift to their Manager: an internal incident report must be completed for the Manager to review: The Manager was responsible in reviewing the incident and reporting to Licensing via phone or email with 24 hours of the incident occurring and; On Fridays, the facility Manager was responsible in ensuring the incident was reported within 24 hours. This corrective action plan was reviewed and accepted by Licensing.
The licensee must record compliance with the care plan of a child requiring extra support in respect of each of the following that are applicable to the child: any behavioural guidance provided to the child, and its effect
Observation: Through the investigation process, Licensing determined that a record of compliance with the care plan of a child requiring extra support in respect of each of the following that are applicable to the child: any behavioural guidance provided to the child, and its effect was not recorded.
Corrective action: Ensure a record of compliance with the care plan of a child requiring extra support in respect of each of the following that are applicable to the child: any behavioural guidance provided to the child, and its effect. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On March 28, 2022, Licensee submitted a corrective action plan to Licensing. This corrective action plan included: the Manager would ensure that any implementation of any modification, guidance and or strategies for a child with a care plan would be recorded in a logbook and all existing and new staff would be informed of the responsibility of documenting any modifications, guidance or strategies to the care plan. This corrective action plan was reviewed and accepted by Licensing.
A licensee must ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult.
Observation: Through the investigation process, Licensing determined that children were not being being continuously supervised.
Corrective action: Ensure that children are supervised at all times by a person who is an educator, an assistant or a responsible adult. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On March 28, 2022, Licensee submitted a corrective action plan to Licensing. This corrective action plan included: children were to be supervised at all times by qualified staff and for emergencies, staff were to notify the site Manager to assist the staff with the situation. This corrective action plan was reviewed and accepted by Licensing.
