3070 - CCLR 55(2)(a) 
 

A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

Observation: Through the investigation process, Licensing determined that an reportable incident had not been reported to Licensing within 24 hours.

Corrective action: Ensure that Licensing is notified of a reportable incident within 24 hours after a child has been involved or may have been involved in a reportable incident as described in Schedule H while under the care or supervision of the Licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 22, 23 & 24, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included that a reportable incident would be reported to Licensing within 24 hours. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.

3020 - CCALA 7(1)(b)(i) 
 

A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

Observation: Through the investigation process, Licensing determined that staff did not demonstrate the skills and abilities to carry out the duties as assigned. Specifically, that staff had not followed polices related to Lockdown Drills/Hold and Secure Policy and Pick-up/Unauthorized Pick-up Policy.

Corrective action: Ensure the community care facility is operated in a manner that will promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On March 22, 23 & 24, Licensee submitted a corrective action plan to Licensing. The corrective action plan included implementation of policies that all parents would be notified via email of operating changes and children records were provided to each room. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.

3070 - CCLR 55(1)(b) 
 

A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child is involved in, or may have been involved in, a reportable incident described in Schedule H.

Observation: Through the investigation process, Licensing determined that a reportable incident had occurred and parents or emergency contacts were not notified of incident.

Corrective action: Ensure that the Licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child is involved in, or may have been involved in, a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 22, 23 & 24, 2023, Licensee submitted a corrective action plan to Licensing. The corrective action plan included a policy would be implemented that all parents will be notified via email immediately if there is any change to our regular operating schedule. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.

3070 - CCLR 57(1) 
 

A licensee must keep current records for each child showing (a) the information set out in subsection (2)

Observation: Through the investigation process, Licensing determined that a child's file information had not been kept current.

Corrective action: Ensure that current records for each child showing the information set out in subsection (2) and the consents referred to in subsection (3) are kept current. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On March 22, 23 & 24, Licensee submitted a corrective action plan to Licensing. The corrective action plan included updates to any changes to the children's records were kept current. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.

Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On March 22, 23, & 24, 2023 Licensee submitted a corrective action plan to Licensing. The corrective action plan included updates to any changes to children's records. The corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.
