Observation: Through the investigation process, Licensing determined Gillybird Nature Academy was named Baby Bird Infant Toddler on the website and an amendment application had not been received by the Licensee to change the name.
Corrective action: Ensure that a medical health officer is immediately notified of any change in the information provided under Section 9. Actions/s Required by Licensee: Licensee to submit a corrective action plan specific to the contravention. Action/s Required by Licensing: On November 8, 2021, Licensee submitted a corrective action plan to Licensing. This corrective action plan included continuing to operate as Gillybird Nature Schools and informing Licensing of any name change decided on in the future. This corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor this corrective action plan.
