A licensee must ensure that behavioural guidance is appropriate to the age and development of the child who is receiving the guidance,
Observation: Through the investigation process, Licensing determined that a staff did not demonstrate behavioural guidance strategies appropriate to the age and development of the child.
Corrective action: Ensure that behavioural guidance was appropriate to the age and development of the child who was receiving the guidance. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On August 21, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved was terminated. Licensing reviewed and accepted the corrective action plan. On June 30, 2023 Licensing received confirmation the staff was no longer working at the facility.
A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care
Observation: Through the investigation process, Licensing determined a staff did not demonstrate the character and skills necessary to carry out the duties assigned.
Corrective action: Ensure to operate the community care facility in a manner that would promote the health, safety and dignity of persons in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On August 21, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved was no longer working at the facility. Licensing reviewed and accepted the corrective action plan. On June 30, 2023 Licensing received confirmation the staff was no longer working at the facility.
A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,
Observation: Through the investigation process, Licensing determined a reportable incident was not reported to Licensing within 24 hours.
Corrective action: Ensure to notify the medical health officer within 24 hours after (a) a child was involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On August 21, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: all staff would review the Incident Reporting Policy at the September 5th, 2023, staff meeting and all School Age staff and supervisors would attend a Licensing Info Session on incident reporting on September 11, 2023. Licensing reviewed and accepted this corrective action plan. On September 11, 2023, Licensing conducted an Incident Reporting Info Session, where all BGCCVI Fifth St supervisors were present. The facility was currently Not Operating, therefore there were no current staff to attend. On September 15, 2023, meeting minutes of the September 5, 2023, staff meeting were submitted to Licensing and reviewed.
A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child is involved in, or may have been involved in, a reportable incident described in Schedule H.
Observation: Through the investigation process, Licensing determined parents were not immediately notified their child was involved in a reportable incident.
Corrective action: Ensure to immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child was involved in, or may have been involved in, a reportable incident described in Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On August 21, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: all staff would review the Incident Reporting Policy at the September 5th, 2023, staff meeting and all School Age staff and supervisors would attend a Licensing Info Session on incident reporting on September 11, 2023. Licensing reviewed and accepted this corrective action plan. On September 11, 2023, Licensing conducted an Incident Reporting Info Session, where all BGCCVI Fifth Street supervisors were present. The facility was currently Not Operating, therefore there were no current staff to attend. On September 15, 2023, meeting minutes of the September 5, 2023, staff meeting were submitted to Licensing and reviewed.
A licensee must ensure that a child, while under the care or supervision of the licensee, is not subjected to any of the following: (c) harsh, belittling or degrading treatment by an employee or another child, whether verbal, emotional or physical, that could humiliate the child or undermine the child’s self respect
Observation: Through the investigation process, Licensing determined a staff grabbed a child by the wrist to stop them.
Corrective action: Ensure that a child, while under the care or supervision of the licensee, was not subjected to any of the following: (c) harsh, belittling or degrading treatment by an employee or another child, whether verbal, emotional or physical, that could humiliate the child or undermine the child's self respect. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to this contravention. Action/s Required by Licensing: On August 21, 2023, the Licensee submitted a corrective action plan to Licensing. The corrective action plan included: the staff involved was no longer working at the facility. Licensing reviewed and accepted the corrective action plan.
