3010 - CCALA 7(1)(b)(i) 
 

A licensee must do all of the following:(b) operate the community care facility in a manner that will promote the health, safety and dignity of persons in care

Observation: Through the investigation process, Licensing determined that the Licensee and one staff had not demonstrated the skills and abilities to carry out the assigned duties. Specifically, the Licensee and one staff had carried out duties without a care plan and without the consultation of the parent.

Corrective action: Ensure that the Licensee and staff demonstrated the skills necessary to carry out all duties assigned. Action/s Required by the Licensee: Licensee to submit a corrective action plan specific to the contravention. Action/s Required by Licensing: On June 17, 2021 the Licensee submitted a corrective action plan specific to the contravention. The corrective action plan included the Licensee and all staff were aware a care plan was in place and to consult with the parent prior to providing extra support. This corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor the corrective action plan. Follow Up Inspection required. On July 27, 2021 Licensing reviewed the corrective action plan with the Licensee. Action/s Required by Licensing: No further action required: this contravention has been addressed.

3070 - CCLR 55(1)(b) 
 

A licensee must immediately notify a parent or emergency contact if, while under the care or supervision of the licensee, the child is involved in, or may have been involved in, a reportable incident described in Schedule H.

Observation: Through the investigation process, Licensing determined that the Licensee had not immediately notified a parent of a incident involving the child in care.

Corrective action: Ensure that while under the care of the Licensee, that a parent of a child in care is immediately notified if the child was or may have been involved in a reportable incident under Schedule H. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 17, 2021 the Licensee submitted a corrective action plan specific to the contravention. This corrective action plan included the Licensee meeting with staff and reviewed the need to immediately notify parents and document the notification regarding reportable incidents. This corrective action was reviewed and accepted by Licensing. Licensing to review and monitor the corrective action plan. Follow Up Inspection required. On July 27, 2021 Licensing reviewed the corrective action plan with the Licensee. Action/s Required by Licensing: No further action required; this contravention has been addressed.

3070 - CCLR 55(2)(a) 
 

A licensee must notify the medical health officer within 24 hours after (a) a child is involved in, or may have been involved in, a reportable incident described in Schedule H while under the care or supervision of the licensee,

Observation: Through the investigative process, Licensing determined that the Licensee had not notified the medical health officer within 24 hours of a reportable incident while a child had been under the care and supervision of the Licensee.

Corrective action: Ensure that the medical health officer was notified within 24 hours of any reportable incident involving children in care. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 17, 2021 the Licensee submitted a corrective action plan to Licensing specific to the contravention. The corrective action plan included that the Licensee met with staff regarding incident reporting requirements. This corrective action plan was reviewed and accepted by Licensing. Licensing to review and monitor the corrective action plan. Follow Up Inspection required. On July 27, 2021 Licensing reviewed the corrective action plan with the Licensee. Action/s required by Licensing: No further action required; this contravention has been address.

3030 - CCLR 52(2)(a) 
 

A licensee must ensure that a child is not, while under the care or supervision of the licensee, subjected to any of the following types of abuse or neglect: emotional abuse,

Observation: Through the investigation process, Licensing determined that a staff subjected a child in care to emotional abuse. Specifically that a staff had yelled at a child in care.

Corrective action: Ensure that while under the care of supervision of the Licensee, that no child was subjected to any type of abuse. Action/s Required by Licensee: Licensee to submit a corrective action plan to Licensing specific to the contravention. Action/s Required by Licensing: On June 17, 2021 the Licensee submitted a corrective action plan specific to the contravention. The corrective action plan included the Licensee had met with the staff to ensure they were aware that yelling at a child was not permitted unless raising the caregiver's voice was part of a care plan. This corrective action was reviewed and accepted by Licensing. Licensing to review and monitor the corrective action plan. Follow Up Inspection required. On July 27, 2021 Licensing conducted an inspection to review the corrective action plan with the Licensee. Action/s Required by Licensing: No further action required: this contravention has been addressed.
