Comments
The purpose of this second visit is at the request of the operator in regard to the conditions that led to the health permit suspension. No rodent activity was observed. The gap located at the bottom corner of the entry door was observed to be enclosed with steel wool. Continue to eliminate the gap with a non-removable and easily cleanable material that will be reviewed at the time of the follow up inspection. The health permit has been reinstated. A one week follow up inspection has been scheduled.
