36: 1320 (A-D) Did not observed an ambient air thermometer inside of the cold drawers Corrective Actions: Instructed the PIC to obtain an ambient air thermometer inside of the cold drawers to ensure proper cold holding temperatures
38: 3270 (3) Observed German cockroach droppings on the wall corners near the behind the Avantco prep cooler. Corrective Actions: Instructed the PIC to clean the droppings to prevent German cockroaches from harboring
38: 3280 Observed 1 live (inside of the Dimchae chest cooler) and 1 dead (inside of the cold drawers) German cockroach(es) in a few areas in the kitchen Corrective Actions: Instructed the person to contact a pest control operator to treat for German cockroaches and any other pests throughout the kitchen. The PIC was also instructed to remove all food items where the roaches were found and to clean and sanitize them before storing more food items there.
38: 3280 Observed rat droppings (seemed to be roof rat droppings) underneath the dishmachine Corrective Actions: Instructed the PIC to clean all rat droppings and contact their pest control operator to treat for rats and any other pests throughout the kitchen
38: 2930 Observed spacing in the sides and bottom of the back door Corrective Actions: Instructed the PIC to seal all openings to prevent the entrance of pests
55: 3180 Observed a mold like substance on the back wall in between the two GE homestyle refrigerators Corrective Actions: Instructed the PIC to clean the wall of the substance
55: 2810 Observed missing tiles in front of the stove top, as well as exposed rough concrete in between the cooking range and 3 compartment sink Corrective Actions: Instructed the PIC to replace all missing tiles and repair the rough concrete to ensure all floors are smooth and easily cleanable
