Observations & Corrective Actions

1: 0060 (1) (2) (3a, d-p) Observed that PIC lacked sufficient knowledge through the demonstration of an inability to properly maintain equipment and the kitchen environment.
Corrective Actions: PICs should have the proper knowledge to ensure that conditions are not conducive to the development of foodborne illness. EHS advised PIC that they should gain more knowledge on the effective cleaning and sanitization of the kitchen equipment and environment.

1: 0070 (1-14) & (16)) Observed that there were deplorable conditions in the establishments kitchen.
Corrective Actions: It is the duty of the PIC to train employees to maintain proper health standards. EHS advised PIC that they are responsible for the training of food service employees and maintaining the conditions of the cooking environments.

2: 55 Observed that the facility has not designated a PIC that is a Certified Food Protection Manager from the initial inspection (3/28/24).
Corrective Actions: EHS received guidance from the Food Program Supervisor and has conveyed to the current PIC that they have two weeks from this follow-up inspection to designate and provide a CFPM.

38: 3280 Observed multiple dead cockroaches in the establishments kitchen.
Corrective Actions: All dead pests should be removed as they are found. EHS informed PIC that they must remove pests from the premises.

38: 3270 (1) (2) (4) Observed that conditions inside of the kitchen were conducive to a pest harborage.
Corrective Actions: The facility should be maintained in a way that prevents the harborage of pests. EHS informed PIC that the kitchen needs to be maintained to prevent harborage conditions.

49: 1770 (C) Observed that the non-food contact surfaces of the restaurants kitchen are heavily soiled with grease and debris. The affected items are the sides of all of the equipment, the bottoms of the food prep surfaces, the pipes for the hand sink, and the interior of the roof space.
Corrective Actions: All non-food contact surfaces must be properly cleaned and sanitized. EHS informed designated PIC that they must be properly cleaned.

55: 2790 Observed that storage area floors, ceiling, and walls do not meet standards.
Corrective Actions: All floors, ceilings, and walls must be easily cleanable and safe. EHS informed PIC that their storage area does not meet standards.

55: 3180 Observed that floors, walls, and prep areas are not cleaned at the proper frequency.
Corrective Actions: All floors, walls, and prep areas must be maintained and properly cleaned. EHS informed PIC that they must clean at the proper frequency.

55: 2860 Observed exposed bricks in the kitchen area.
Corrective Actions: Unapproved coverings and coating are not allowed to be used. EHS informed PIC to call in for service.

55: 2880 Observed joists and rafters inside of the storage area above the kitchen exposed. Observed screws protruding from the ceiling in the storage area posing a potential hazard.
Corrective Actions: Joists and rafters are not allowed to be exposed. EHS informed PIC that storage area does not meet standards.

55: 3170 Observed restaurants doors, kitchen tiles, coving, and ceilings were in disrepair.
Corrective Actions: All physical facilities must be maintained in good condition. EHS informed PIC that they must do maintenance on the indicated areas of the building.

55: 2820 Observed exposed utility lines in the kitchen area.
Corrective Actions: Utility lines are not allowed to be exposed in a kitchen area. EHS informed PIC that they must cover exposed utility lines.

Additional Comments

Joint inspection conducted with EHS Sr. Daniel Malones, EHS Chance Clark, and EHS Kayla Hagerman. EHS's followed up on kitchen conditions and overall cleanliness. Two live roaches were identified inside of the kitchen of the establishment. EHS's made sure to inform the current PIC that the kitchen was in need of attention. Equipment and surfaces are not being cleaned at the proper frequency or as needed. Equipment is not being maintained properly and is need of maintenance. The establishment has failed to replace the necessary ceiling tiles leading to a build up grease in the attic area. This is a potential fire hazard and serves as an attractant to pests. EHS informed PIC that they must attend to the cleaning requirements of the kitchen. EHS has provided the owner with two paper permit application, one on a visit on 4/5/2024 and one this visit. EHS has also emailed the permit form multiple times. EHS informed the PIC that non-renewal of the health permit could lead to enforcement actions.
