16: 1770 (A) Observed slicer stored unclean. Food contact equipment shall be thoroughly cleaned after use and stored clean.
Corrective Actions: Food contact equipment shall be thoroughly cleaned after use and stored clean. Thoroughly clean slicer and maintain clean when not in use.

23: 830 Observed the following packaged food items, not intended to be consumed within 24 hours, removed from original container and not bearing date and name label: sliced ham, sliced turkey, sliced salami, and prepared pasta.
Corrective Actions: Refrigerated ready-to-eat, time/temperature control for safety food prepared and packaged by a food processing plant or foods prepared by the food establishment shall be clearly marked at the time the original container is opened in a food establishment or prepared by the food establishment, and if the food is held for more than 24 hours, to indicate the date or day by which the food shall be consumed on the premises, sold, or discarded, based on the temperature and time combinations.

39: 610 Observed prepared food stored unprotected. Containers of sliced onion, diced cooked chicken, sliced meatballs and cheese stored uncovered on the bottom shelves in the pizza prep coolers. Containers of feta cheese and onions stored directly on the floor in the walk-in-cooler. Bread and garlic knots stored directly in plastic grocery bags. Food shall be stored protected from contamination.
Corrective Actions: Food shall be stored protected from contamination. Elevate food off of floor and cover prepared foods. Foods elevated off floor and covered with plastic wrap. Bread and garlic knots stored in grocery bags voluntarily discarded.

49: 1800 Observed cart holding clean in place slicer and containers of clean wares, reach in freezer bottom shelf, and walk in cooler fan grates unclean.
Corrective Actions: Equipment shall be maintained clean. Clean areas.

55: 3170 The following areas of the physical facilities observed in disrepair: walk-in-cooler floor is cracked and bowed, and poses as a potential hazard, the right-side door gaskets of the reach-in-cooler are not tightly fitted, ice particles have formed surrounding the door.
Corrective Actions: The physical facilities shall be maintained in good repair. Repair areas and maintain.

Notes: Ceiling tiles above walk-in-cooler and above the 3 compartment sink not in place -Replace tiles.
Do not use cardboard as shelf liner.
