28: 3430 Observed two bottles of Raid "Ant and Roach/Flying Insect" labeled for residential use only in back of house. Observed "Mice Rodent Defense" spray in the back of house. Corrective Actions: Raid discarded, please ensure that pesticides labeled "Residential Use Only" are not used in establishment.
35: 790 Observed frozen raw chicken breast thawing in back of house at ambient temperatures. Corrective Actions: Please thaw per methods approved by code, inspector discussed with MGMT.
36: Observed the ambient air thermometer in the 2 dr prep top to be broken. Corrective Actions: Replace.

38: 3280 Observed 2 glue traps containing dead roaches. Corrective Actions: Removing dead or trapped birds, insects, rodents, and other pests.

47: 1570 Observed the entire bottom seal to be missing from the walk in door. Unit is maintaining temperature, but leaking air. Corrective Actions: Repair door seal to prevent loss and maintain proper repair as intended.
51: 2350 (ii) Observed leaking from hand sink in the back of house, observed leaking from 3 VAT sink. Corrective Actions: Maintain system in good repair.
55: 3220 Observed two soiled mops stored in buckets with stagnant grey water in the back of house. Corrective Actions: Please allow for proper draining.
55: 3180 Back of house prep area noted in need of cleaning. Corrective Actions: Please maintain frequency.
